Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.

• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Provides service within the scope of practice and plan of care.
• Evaluates patient functionality and CMS documentation needs; coordinates with physician.
• Observes, records, and reports patient response to treatment and changes to condition.
• Conducts patient assessments using functional ability tests.
