Job summary

St. David's Healthcare at Home in Texas is seeking a PRN Home Health LPN/LVN to deliver compassionate, patient‑centered care in patients' homes. You will work with RNs and the care team to manage medical needs, support independence, and visit patients on an as-needed basis.

Responsibilities include wound care, med administration, assessments, and education for patients and families. You will document care, monitor changes, and coordinate with providers to ensure safe, quality home care.

Qualifications

• Licensed Practical or Vocational Nurse (LPN/LVN) with licensure in good standing.
• Driver License (DL) required.
• Vocational School Graduate.

Responsibilities

• Provide nursing care in the home, including wound care, med administration, and support during procedures.
• Document assessments, interventions, and outcomes clearly to support safe, continuous care.
• Monitor patient status and alert the care team of any changes for timely responses.
• Educate patients and families to support medication adherence and understanding of health conditions.
• Collaborate with RNs, providers, therapists, and care coordinators to deliver high-quality home care.
