Job summary

Methodist Hospitals is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services in patients' homes under RN supervision. You will conduct assessments, administer medications, perform wound and catheter care, and document visits in EMR/EVV systems within 24 hours.

The role requires Indiana LPN licensure, BLS certification, and strong communication and organizational skills. Driver’s license and reliable transportation are necessary for home visits.

Qualifications

• Graduate of a state-approved practical nursing program.
• Current, unrestricted Indiana LPN license or multi-state license with Indiana authority.
• One year of medical-surgical, skilled nursing, home-health, or related nursing experience preferred.
• Current Basic Life Support (BLS) certification required.
• Valid driver’s license, reliable transportation, and automobile insurance meeting agency requirements.
• Completion of agency orientation and demonstration of required home-health clinical competencies.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction and established plan of care.
• Collect and document focused patient information; monitor and report changes in condition.
• Administer medications and treatments within LPN scope of practice and policies.
• Perform wound care, catheter and drain care, specimen collection, and patient monitoring.
• Assist RN or practitioner with specialized procedures and prepare equipment using aseptic techniques.
• Reinforce education to patients and families on disease management and self-care.
• Maintain documentation in EMR/EVV within 24 hours of visits.
• Maintain patient confidentiality and adhere to HIPAA and regulatory requirements.
