Job summary

Qualifications

• Current nursing license in the practicing state.
• Two years experience as an LPN/LVN in a clinical setting.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction.
• Implement individualized care plans with patient, family, and healthcare team.
• Educate patients and families on disease management and self-care.
• Maintain accurate records of patient care, visits, interventions, and outcomes.
• Coordinate care and communicate patient status with the healthcare team and physicians.
