Qualifications

• Valid LPN license in good standing.
• Proven experience in home health preferred.
• Excellent knowledge of nursing care methods.
• Familiarity with health and safety guidelines and procedures.

Responsibilities

• Chart observations of the patient's condition during visits.
• Administer physician-prescribed medications.
• Dress or redress wounds and monitor healing progress.
• Educate patients and families on home health care procedures.
• Coordinate with physicians and therapists to support the care plan.
