Job summary

Caretenders Home Health, a part of LHC Group, is seeking a Licensed Practical Nurse (LPN) to deliver high-quality in-home patient care under the plan of care.

Applicants must have current LPN licensure, a valid driver’s license, reliable transportation, and the ability to multitask with strong communication skills. CPR certification preferred and adaptable to flexible hours as needed.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Valid driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice in home health per plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician and team.
• Function in diverse home environments and demographics.
