Job summary

Reliant at Home seeks a PRN Home Health and Hospice Caregiver to provide personal healthcare services to patients in Lufkin and surrounding areas.

Primary responsibilities include supporting patients in their homes, maintaining a safe environment, and reporting observations to the supervising RN. Requires a High School Diploma and a valid driver’s license, with at least one year of Home Health Aide or Nursing Assistant experience.

Qualifications

• High School Diploma preferred.
• Must have a current driver's license.
• At least 1 year of Home Health Aide or Nursing Assistant experience in hospital, nursing home, or home health agency.

Responsibilities

• Provide personal healthcare and related services in the patient’s home.
• Assist in maintaining a safe and clean home environment.
• Report observations and problems to the supervisor.
