Job summary

Humana is seeking a Home Health Aide to deliver compassionate care in patients' homes under RN supervision. The role focuses on rehabilitation and functional improvement, helping patients regain strength and independence for daily activities.

Responsibilities include direct patient care, vital signs, medication observation, hygiene support, nutrition guidance, mobility assistance, and documentation to ensure continuity of care.

Qualifications

• Complete CNA or HHA program within last 24 months.
• Meet applicable state certification requirements.
• HHA/CNA course to work for a Medicare-certified agency.

Responsibilities

• Provide direct patient care under RN direction and according to the Aide Plan of Care.
• Assist patient with self-administered medications and monitor dosages.
• Take and report vital signs like temperature, pulse and blood pressure.
• Support personal hygiene and grooming activities.
• Assist with feeding, document intake and help with meals.
• Assist with toileting and ostomy care as needed.
• Support mobility, positioning, transfers and ambulation per Plan of Care.
• Document care and changes to ensure continuity of care.
• Practice infection control and maintain safe environment.
• Assist with light household tasks when needed and allowed by MD order.
