CenterWell Home Health in Andalusia, AL is seeking a Home Health Aide (HHA) to provide direct patient care under the supervision of an RN and per the Plan of Care. You will assist with medications, monitor vital signs, and support hygiene, mobility and daily activities in the patient’s home.

Requirements include a high school diploma or equivalent, a CNA/HHA program completed within 24 months, valid state HHA/CNA certification, and a reliable vehicle with insurance.

Qualifikationen

• High school diploma or equivalent is required.
• Completion of CNA or HHA program within the last 24 months is required.
• Must meet applicable state HHA certification requirements.
• A valid driver's license and auto insurance are required.

Aufgaben

• Provides direct patient care under RN direction and POC.
• Assists with self-administered medications and ensures patient takes medications as prescribed.
• Measures and reports temperature, pulse, and BP; flags abnormal results.
• Assists with hygiene, grooming, nutrition, and feeding.
• Assists with toileting, mobility, transfers, and ambulation per care plan.
• Documents care provided and communicates with patient, caregivers, doctors and staff.
• Maintains infection control standards and a clean environment.
• Supports light household tasks as needed when MD orders are present.
