Job summary

Sunrise Health Services seeks a Certified Nursing Assistant to provide compassionate personal care to patients under the supervision of a nurse or nurse supervisor. You will support hygiene, safety, nutrition, mobility, and essential treatments per care plans.

You will document care activities, report patient changes, and collaborate with the interdisciplinary team to promote comfort and independence for clients in their home settings.

Qualifications

• High School graduate or equivalent preferred.
• Proof of state certification, if applicable.
• One-year direct patient care experience in an institutional setting.
• Ability to travel per job requirements.
• Telephone available in home.

Responsibilities

• Provide personal care and services to the patient as per written assignment.
• Assist with hygiene, environment, meals, and safe mobility.
• Assist with prescribed treatments and medications self-administered as instructed.
• Assist with ambulation and use of devices under nurse supervision.
• Maintain accurate records on care provided and report changes to the nurse.
