Job summary

Adaptive Home Health is seeking a Field LVN/HHA in Texas to provide compassionate in-home personal care and support to patients. The role is full-time, Monday–Friday with some weekends as needed, and pays $17.00–$18.00 per hour plus mileage reimbursement.

You will work field-based, coordinating with the RN/clinical team, using Homecare Homebase for documentation and charting, with a focus on maintaining patient independence and safety.

Qualifications

• Must have Texas HHA certification and CPR/BLS.
• 1+ years of personal care experience in home health, assisted living, or hospital setting.
• Current Texas CNA certificate and Texas driver’s license with reliable transportation.

Responsibilities

• Provide personal care including bathing, dressing, grooming, and toileting.
• Assist with transfers, ambulation, and mobility using proper body mechanics.
• Support with medication reminders as directed by care plan.
