Adaptive Home Health is seeking field-based caregivers to provide compassionate, patient-centered home care across Texas. You will focus on safety and helping patients maintain independence while delivering high-quality care.

Responsibilities include bathing, mobility support, medication reminders, and documenting in HCHB, coordinating with supervising clinicians, and ensuring continuity of care in patients’ homes. Reliable transportation is preferred.

Qualifications

• Current Texas HHA certification.
• 1+ years of personal care experience.
• CPR/BLS certification.
• Texas driver's license and reliable transportation.
• Ability to lift, transfer, and provide hands-on care safely.
• Strong communication skills and professional demeanor.
• Comfortable working independently in patients’ homes.
• High school diploma or GED.
