Job summary

A trusted home care provider is seeking a Home Health Aide to work directly with patients in their homes, offering personalized care and support. The ideal candidate will have a high school diploma or GED and must be a graduate of an accredited Home Health Aide program. Strong communication skills and prior experience in home health or hospice care are preferred. This position offers flexible scheduling and numerous advancement opportunities.

Qualifications

• Must have graduated from an accredited Home Health Aide program.
• Prior Home Health or Hospice experience is a plus.
• Driver’s license is required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor.
• Build relationships with patients by conversing or reading aloud.
• Give medication reminders.
• Engage patients in exercises or other activities.
