Qualifications

• One year prior professional experience in home health or hospice.
• Driver’s license required.
• CPR certification required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds.
• Care for patients by changing bed linens and cleaning the home.
• Maintain records of patient care and report observations.
• Build relationships with patients through conversation.
• Give medications and immunizations.
• Engage patients in exercises and activities.
