Job summary

Summit Home Health Care is seeking caring, trustworthy caregivers in New York to deliver compassionate in-home support. You will perform daily personal care, assist with activities of daily living, and accompany patients to appointments, with emphasis on safety and dignity.

Candidates must hold a New York State Home Health Aide Certificate, provide IDs and work authorization, letters of reference, a recent physical, TB screening, flu shot, rubella and rubeola titers, and a chain of custody drug

Qualifications

• Must have New York State Home Health Aide Certificate.
• Government issued ID, Social Security Card and work authorization if needed.
• 2 recommendation letters and a recent physical.
• TB screening (Quantiferon preferred), flu shot, rubella & rubeola titers.
• Chain of Custody drug screen no older than 6 months.

Responsibilities

• Daily personal care (bathing, grooming, oral care, dressing, etc.).
• Assist with daily living activities per Plan of Care.
• Monitor patient condition and report changes.
• Assist with meals, basic housekeeping, and errands.
• Accompany to doctor's visits, shopping, and essential tasks.
