Job summary

Enhabit Home Health & Hospice is seeking a dedicated Home Health Aide to join our team in a role that makes a daily difference. The position supports clients with daily living activities, basic hygiene, and routine exercises, while following individualized care plans.

Qualifications include a preferred CNA and current Nurse Aide Registry in the state, plus at least one year of home health or hospital/long-term care experience.

Qualifications

• CNA preferred and registered in state Nurse Aide Registry.
• Minimum of 1 year of experience in home health or hospice.
• Minimum of 1 year of direct client care in an institutional setting (hospital or nursing facility).
• Ability to recognize client needs, maintain good relationships, and communicate observations to supervisor.

Responsibilities

• Carry out tasks on clients and follow home health care plans as directed.
• Bathe clients and assist with showers and partial baths.
• Assist with exercises and rehabilitation in coordination with PT when applicable.
• Support client with personal hygiene and activities of daily living.
• Answer questions about self-care and provide family/client instructions.
• Help client prepare for doctor appointments.
• Lift, turn, and weigh clients as needed.
• Record vital signs as assigned (blood pressure, temperature, respiration).
