You will follow each individual's care plan and support residents with daily activities, mobility, and safety. Qualified applicants will have a high school diploma and CT-certified Nursing Assistant or Home Health Aide credentials, a valid CT driver's license, and reliable transportation.

Qualifications

• High School Diploma or equivalent.
• Valid Nursing Assistant certification or Home Health Aide certification listed with CT State Registry.
• Active CT driver's license and reliable vehicle.

Responsibilities

• Follow each client's care plan detailing services.
• Provide personal care and homemaker services to enable independence at home.
• Maintain accurate documentation and operate assistive devices as needed.
