Job summary

Herewith is seeking compassionate In-Home Dialysis Care Partners for Oklahoma, 74901. You will support patients receiving dialysis at home, ensuring safe environments, prepping for treatment, and monitoring wellbeing. NxStage dialysis experience preferred; 1 year minimum.

You will be matched with patients based on skill fit and personality, working with a clinical team to provide reliable care and flexible scheduling in a home setting.

Qualifications

• At least 1 year of NxStage dialysis machine experience.
• Prior experience supporting dialysis patients (home or clinical).
• Reliable, empathetic and detail-oriented with strong communication.

Responsibilities

• Conduct and support patient setup for dialysis treatments (NxStage or equivalent).
• Provide companionship, emotional reassurance, and schedule adherence.
• Monitor safety and assist with light ADLs.
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during support hours.
