Job summary

Herewith is seeking compassionate In-Home Dialysis Care Partners to support patients in their homes across Chicago. You will ensure a safe environment, assist with treatment prep, and monitor overall wellbeing while coordinating with a clinical team.

The role emphasizes dependable, empathetic care with consistent weekly hours and full placement support from Herewith. Prior dialysis experience is preferred, and background checks are mandatory.

Qualifications

• 1+ year hands-on dialysis machine experience.
• Experience supporting dialysis patients in home or clinical settings.
• Reliable, empathetic, and detail-oriented.
• Strong interpersonal communication.
• Background check and vetting via Herewith platform required.

Responsibilities

• Set up dialysis treatments and assist patient setup (NxStage, Tablo or equivalent).
• Provide companionship, emotional reassurance, and schedule adherence.
• Monitor safety and assist with light activities of daily living (ADLs).
• Maintain communication with clinic nurses and care coordinators.
• Document concerns or changes observed during support hours.
