Job summary

Qualifications

• Compassionate and dependable caregiver with a patient, caring approach.
• Ability to follow individualized care plans and document changes.

Responsibilities

• Assist with personal hygiene (bathing, grooming, oral care).
• Assist with dressing and undressing.
• Support toileting and incontinence care as needed.
• Aid mobility and transfers between beds and chairs per care plan.
• Prepare meals and assist with feeding when needed; encourage hydration.
• Provide medication reminders per established routine.
• Offer companionship and social engagement throughout the day.
• Perform light housekeeping to maintain a safe home environment.
• Monitor client condition and report changes to the care team.
