Job summary

Celestial Care is seeking a compassionate Personal Care Worker to join our home care team in South Milwaukee, WI. You will implement care plans, assist with daily living activities, and report changes to the RN Supervisor.

Responsibilities include recording client information, meal preparation, light housekeeping, transportation, and providing companionship. A minimum of one year of PCW experience and reliable transportation are required.

Qualifications

• Minimum of one year of personal care experience.
• Must be at least 18 years old.
• Must be physically able to perform duties.
• Must maintain patient confidentiality and follow directions from RN Supervisor.
• Reliable transportation to assignments.

Responsibilities

• Record pertinent information about the client’s care.
• Remind clients to take medications.
• Plan and prepare meals per dietary needs and assist feeding if needed.
• Assist with personal care and grooming.
• Assist with ambulation and mobility.
• Assist with routine bodily functions (toileting).
• Perform light housekeeping and laundry.
• Provide companionship and accompany to appointments.
• Run errands as needed.
• Follow care plan and report changes to RN Supervisor.
• Prepare daily records and submit to the office by due date.
• Participate in in-service education programs.
