Job summary

Live Well Home Care in Wisconsin is seeking compassionate caregivers to provide in-home support to clients who require assistance with daily living due to age, illness, or disability. You will perform personal care, meal prep, light housekeeping, transportation, and companionship to improve quality of life.

You will follow care plans, monitor safety, document observations, and coordinate with the Branch Manager to ensure respectful, person-centered care in the client's home.

Responsibilities

• Assist with activities of daily living and personal care including bathing, dressing, grooming, toileting, transferring, and mobility.
• Provide companionship, social interaction, and emotional reassurance to clients in their homes.
• Assist with meal planning, meal preparation, and routine housekeeping such as cleaning, laundry, and laundry duties.
• Escort clients to appointments, social activities, and community events as specified in the care plan.
• Ensure client safety by monitoring the home environment and reporting hazards to the supervisor.
• Assist with medication reminders and following a written care plan.
• Support clients with transportation and errands while respecting client independence and preferences.
• Document care provided and changes in client condition in accordance with policies.
