Qualifikationen

• Experience in home & community care, assisted living, nursing homes, or hospice care.

Aufgaben

• Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and mobility support.
• Monitor and record vital signs such as blood pressure, pulse, and temperature.
• Give medication reminders following care plans and HIPAA guidelines.
• Provide meal preparation and nutritional support tailored to individual dietary needs.
• Support patients with memory care, dementia, Alzheimer's, or other cognitive impairments.
• Use assistive devices to ensure safe transfers and mobility.
• Observe and report changes in patient condition or behavior to care team.
• Perform light housekeeping and follow care plans.
• Document services provided accurately and support patients during social activities.
