Job summary

Choices By Design LLC is seeking a compassionate Home Care Aide to support patients in their homes with one-on-one care plans. You will be directly caring for clients, building genuine relationships, and maintaining a healthy and safe home environment with attention to detail and clear communication.

Candidates with prior Home Health or Hospice experience are a plus; strong verbal and written communication skills are essential to success in this role, along with reliable, organized conduct.

Qualifications

• Experience in Home Health or Hospice is a plus.

• Detail-oriented and organized.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
