Job summary

Bluebonnet Health Services is seeking a Registered Nurse to provide nursing care to patients in their homes and coordinate with the interdisciplinary team. You will ensure timely assessments, develop and implement individualized care plans, and educate patients and families about end-of-life care.

The role requires an RN license, 2+ years of clinical RN experience, and hospice familiarity. Strong English skills, reliability, and transportation are essential for home-based visits.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license; Nurse Licensure Compact eligibility.
• Two years RN experience in a clinical care setting; Hospice experience preferred.
• Must read, write and comprehend English.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs and impact of terminal disease.
• Develop, implement, and document care plans for quality and continuity of care.
• Educate patient and family on disease process, self-care, end-of-life care, nutrition, and needs.
• Provide clinical directions to the Hospice Aide and LPN/LVN; train staff as needed.
• Maintain progress notes and communicate changes to physicians and team members.
• Participate in Hospice QAPI programs and on-call responsibilities.
