Qualifications

• Valid HHA or PCA Certificate.
• Valid Photo ID.
• Social Security Card.
• Full Physical exam form, PPD, Rubella & Rubeola titers within 6 months.
• Drug Screen 8-10 panel within 6 months.
• Two References letters within 6 months.
