Qualifications

• 1 year of experience as a Home Health Aide or similar role.
• Ability to perform personal care and daily living activities.
• Reliable transportation and safe lifting ability.
• Strong communication and interpersonal skills.

Responsibilities

• Provide in-home care and support to clients with daily living activities, including personal care, meal preparation, and light housekeeping
• Assist clients with medication management, wound care, and other health-related needs
• Monitor and report changes in client's condition to the healthcare team
• Collaborate with clients, families, and healthcare providers to develop and implement personalized care plans
• Maintain accurate and detailed records of client care and services provided
