Job summary

Trilogy Health Services, LLC in Kokomo is seeking a Certified Assisted Living Associate (HHA) to provide care and support to residents, ensuring their comfort, dignity, and safety. You will perform direct patient care and work with healthcare professionals to maintain a positive environment for residents.

Responsibilities include assisting with daily living activities, monitoring vital signs, offering companionship, and helping with meals and mobility. Indiana HHA certification is required.

Qualifications

• High School Diploma or GED/HSE preferred
• 0-1 Years of relevant experience preferred
• Indiana Home Health Aide Certification Required (HHA)

Responsibilities

• Assists AL residents with daily living activities, including bathing, dressing, grooming, toileting, and mobility.
• Monitors and documents vital signs as directed by supervising nursing staff.
• Provides companionship and emotional support to residents.
• Assist AL residents during mealtimes, including serving food and ensuring dietary needs are met.
• Helps with mobility and transfers, using proper body mechanics and safety protocols.
• Maintains a clean, safe, and organized environment in residents’ rooms and common areas.
• Complies with facility policies, state regulations, and safety guidelines.

Education
