Qualifications

• Eighteen years of age or older.
• Pass background check.
• Driver's license required.
• High school diploma/GED required.
• Valid auto insurance required.

Responsibilities

• Perform personal care tasks including hygiene, feeding, ambulation and health monitoring.
• Foster positive relationships with individuals served and their support network.
• Encourage self-help activities.
• Implement Individual Service Plan and Behavior Support Plans.
• Manage behaviors by prevention or de-escalation of aggression.
• Keep accurate daily documentation of activities, medications and data tracking.
• Transport individuals to community activities and medical appointments.
• Perform other duties as assigned.
