Job summary

Journey Found is seeking a Direct Support Specialist in Manchester, CT to assist individuals with daily living, accompany them to appointments, and help manage routines in group homes. The role involves medication administration, documenting data, and supporting community activities, with a strong emphasis on teamwork and compassionate care.

The position offers comprehensive benefits and training, including CPR/First Aid and MAP completion, with a focus on safety and person-centered planning.

Qualifications

• High school diploma or equivalent required.
• At least 3 years of driving experience with valid license.
• Behavioral management experience or formal training preferred.
• Ability to communicate clearly, both oral and written.
• PMT, CPR, FA and DDS Medication Administration Program (MAP) required.
• Must be free from communicable disease.

Responsibilities

• Assist individuals with activities of daily living (dressing, cooking, cleaning, bathing, toileting).
• Transport individuals to activities and appointments.
• Organize activities and community outings.
• Administer medications per plan.
• Attend doctors' appointments with individuals.
• Document program data accurately.
• Teach and engage individuals in activities.
• Maintain routines and promote stability.
• Assist in decision making processes.
• Grocery shopping and running errands.
• De-escalate situations and manage behaviors.
• Work as part of a collective team.
• Communicate professionally with Nurses, Clinical Staff, Directors and Supervisors.
