Qualifications

• Valid health certificate to meet funding requirements.
• Ability to move/adjust at least 75 pounds and assist with transfers.
• Valid driver’s license and clean driving record.
• Ability to follow care plans and maintain confidentiality.

Responsibilities

• Provide hands-on assistance in daily living activities.
• Assist with meals, laundry, and household chores.
• Follow Positive Behavior Supports and safety protocols.
• Attend PCP meetings and document logs in agency software.
• Support independence using person-centered planning.
