Job summary

Pathway 2 Hearts LLC is seeking Direct Support Staff/Caregivers to support residents in an Adult Family Home setting in Wisconsin. You will assist with daily living activities, promote independence, and help monitor health and well-being.

Role requires empathy, patience, and strong communication. Prior caregiving experience and CPR/First Aid certification are preferred; a valid driver's license is helpful for transportation needs.

Qualifications

• High School Diploma or equivalent.
• Previous experience in a similar role or relevant training is preferred.
• CPR and First Aid certification may be required.
• Ability to show empathy, patience, and compassion.
• Driver's License and reliable transportation.

Responsibilities

• Assist residents with activities of daily living, including bathing, grooming, dressing, and toileting.
• Provide mobility support and help with transfers if needed.
• Administer medications as prescribed, ensuring compliance with healthcare plans.
• Monitor and document any side effects or changes in residents' health.
• Engage residents in meaningful activities to promote socialization and recreational participation.
• Assist with meal preparation and ensure dietary needs are met.
• Observe and report changes in health or behavior to healthcare professionals.
• Follow protocols for emergency situations and provide first aid when necessary.
• Maintain accurate care records and complete required documentation for regulatory compliance.
• Collaborate with staff, healthcare professionals, and families to ensure comprehensive care.
• Communicate effectively with residents to understand their needs and preferences.
• Ensure a safe and clean living environment and follow safety protocols.
• Advocate for residents' rights and independence.
