Qualifications

• GED or high school diploma required.
• Basic reading and writing skills to follow instructions.
• Knowledge of dementia and elder care preferred.
• Ability to read and follow simple instructions and memos.

Responsibilities

• Assist residents with daily living activities, including bathing, dressing and meals.
• Maintain resident records and document changes in health or behavior.
• Support engagement and social activities per care plan and Life Story.
• Follow medication procedures as permitted by state regulations.
• Communicate clearly with residents, families and team members.
