Job summary

Phoenix Senior Living is hiring a Resident Care Associate (CNA) to provide high-quality care for residents in our Phoenix, AZ facility. You will follow Personalized Service Plans, support daily living activities, and maintain a safe, home-like environment for residents and their families.

The role involves hands-on care, wellness elements, and adherence to regulatory standards. Weekend availability and compassionate, patient-centered service are essential.

Qualifications

• High School education preferred or required by the state.
• Experience working with elders or disabled individuals preferred.
• Must be at least 18 years old to perform personal care tasks.
• Knowledge of Assisted Living and Dementia care preferred.
• Ability to work weekends, evenings, and flexible hours.

Responsibilities

• Participates in developing and updating Personalized Service Plans; attends plan review meetings with families.
• Follows hydration schedule for residents during the shift.
• Completes meal attendance sheets and coordinates room service when needed.
• Knows and complies with resident rights.
• Initiates and assists with activities as appropriate.
• Checks residents daily for cleanliness, clothing, continence issues, and changes in condition.
• Uses proper sanitary procedures and universal precautions.
• Checks assigned resident rooms daily for cleanliness; double checks high risk rooms.
• Assists with laundry rooms and common areas; handles hazardous chemicals appropriately.
• Completes all ADLs per PSP and responds promptly to security systems and call bells.
