Job summary

Lineage Health Care Consulting is seeking a Certified Nursing Assistant (CNA) to provide outstanding patient care and daily living support for patients and residents. The role requires an Illinois state CNA certification and a high school diploma or equivalent, with CPR current.

You will perform essential duties including assisting with daily activities, monitoring vitals, documenting intake/output, and communicating with physicians and nurses to ensure quality care.

Qualifications

• Must be a state-certified Nursing Assistant (CNA) in Illinois.
• Current CPR (American Heart Association).
• Nursing degree from an approved CNA program.
• Ability to think and work independently and communicate effectively in a fast-paced environment.
• Current CNA certification required.
• High School diploma or equivalent.
• Valid identification and basic computer use.

Responsibilities

• Provide exemplary patient care and daily living support.
• Answer calls and assist patients as needed.
• Assist with activities of daily living including bathing, dressing, mobility.
• Turn and reposition bedridden patients.
• Review dietary restrictions and preferences for meals.
• Obtain information from physicians and nurses about conditions and plans.
• Record intake/output and report changes to staff.
• Record vital signs as requested.
• Examine patients for issues requiring care.
• Remind patients to take medications and supplements.
• Note observations to nurses and stay updated on CNA training and facility policy.
