Job summary

Sierra Home Health and Hospice in Las Cruces, NM seeks a Certified Nurse Assistant to provide compassionate, patient-centered home health and hospice care. You will work under a licensed nurse, assisting with ADLs, vital signs, and documentation while supporting patients at home and with families.

Qualifications

• High school diploma or equivalent.
• Completion of state-approved Nurse Aide Training program.
• Current Nurse Aide certification in the state of practice.
• At least one year of experience preferred; home health or hospice experience a plus.
• Ability to follow instructions and document services accurately.
• Strong communication and interpersonal skills with patients and staff.

Responsibilities

• Provide personal care to patients under supervision of a licensed nurse.
• Assist with activities of daily living including bathing, grooming, dressing, mobility, toileting, and feeding.
• Measure and record vital signs including temperature, pulse, respiration, and blood pressure as directed.
• Observe and report changes in patient condition, safety concerns, or care needs promptly to supervising staff.
• Assist with ambulation, transfers, and positioning to promote comfort and prevent complications.
• Maintain accurate documentation of care provided in the EMR in accordance with agency policies.
• Support infection control, safety, and risk management procedures in the care environment.
• Provide emotional support and companionship to patients and families.
• Participate in in-service training and continuing education programs as assigned.
