Job summary

Heritage-Woods-of-Plainfield is seeking a compassionate Certified Nursing Assistant to provide individualized care per each resident’s care plan. You will assist with Activities of Daily Living, promote independence, and maintain residents’ dignity while reporting health changes to the clinical team.

Engage with residents and families warmly and participate in ongoing trainings and meetings. Quality communication and a strong commitment to privacy are essential as you perform additional duties

Qualifications

• High School graduate or GED equivalent.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver care per resident care plans and assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Build compassionate connections with residents and their families to create a positive experience.
• Participate in ongoing learning through seminars, staff meetings, and other trainings.
• Maintain confidentiality and privacy of resident information in all interactions.
• Perform additional duties as assigned.
