Qualifications

• Massachusetts CNA certification required.
• Ability to lift, move and transfer patients.

Responsibilities

• Provide direct patient care (bathing, dressing, grooming).
• Assist with activities of daily living (ADLs) including toileting, transfers, and ambulation.
• Take vital signs and monitor resident condition.
• Assist with meal preparation and snack service.
• Provide emotional support and companionship to residents.
• Maintain accurate and detailed records of resident care.
• Participate in quality improvement initiatives to improve patient outcomes.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
