• High School Diploma or Equivalent is required.

Responsibilities

• Provide and assist with physical care of patients including bathing, hygiene, grooming, toileting, transfers, feeding and mobility.
• Document patient care and observations in real time.
• Communicate changes in patient condition to licensed nurse promptly.
• Assist in dining and feeding tasks and monitor oral intake.
• Transport patients to scheduled therapy appointments and assist in activities.
• Support infection control and ensure patient rights and dignity are preserved.
• Maintain clean and safe patient rooms and report safety concerns.
