Job summary

• Experience delivering activities of daily living.
• Ability to communicate patient care information accurately and timely.

Responsibilities

• Demonstrates knowledge of activities of daily living for the delivery of patient care.
• Continually assesses data regarding patients status and interprets information to identify needs.
• Participates in the application and maintenance of current patient treatment plans.
• Delivers care based on identified needs and as directed by the licensed staff.
• Prioritizes patient care activities to deliver safe, effective care.
• Administers treatments effectively and skillfully.
• Evaluates outcomes of interventions and implements necessary changes to care.
• Assists in discharge planning with the care team.
• Documents data collection, interventions and evaluations accurately, objectively and timely.
• Serves as advocate for patient/families in decisions affecting the plan of care.
