Job summary

Stonebridge at Montgomery in Skillman, NJ is seeking Certified Home Health Aides (CHHAs) to join our team. CHHAs provide care per the patient care plan, monitor and record conditions, and offer companionship under licensed supervision.

Responsibilities include observing health status, assisting with transfers, bathing, meals, and activities, plus maintaining records and delivering emotional support as needed. A High School diploma and NJ CHHA license are required.

Qualifications

• High school diploma or equivalent is required.
• Valid NJ CHHA license is required.

Responsibilities

• Observe health conditions, progress, or issues and maintain daily records.
• Guide patients in prescribed exercises per care plan.
• Assist with transferring, bathing, grooming and dressing.
• Maintain patient care records and report progress to supervisor.
• Change bed linens and clean patient quarters.
• Entertain or read aloud to patients and provide emotional support.
• Serve meals per prescribed diets and assist with meals as needed.
• Encourage or accompany patients to activity programs.
