Job summary

The Crossings at North River is seeking a dedicated Nursing Assistant to support residents with daily living activities, monitor health conditions, and document care per ISP. You will work closely with the nursing team to ensure safety and comfort.

Ideal candidates hold a CNA certification, CPR/First Aid, and are physically able to lift and assist residents. Competitive pay and team-oriented environment are offered.

Qualifications

• High school diploma or GED is required.
• Nursing Assistant Training Program certificate (State Board approved) may be required.
• Certified Nursing Assistant (CNA) certification preferred.
• CPR and First Aid certifications are required or preferred as noted.
• Must be physically capable of lifting up to 50 pounds and assisting residents.

Responsibilities

• Follow individualized plan of care (ISP) for each resident; observe changes and report to the nursing supervisor; document daily care and red flags, especially night rounds.
• May check and record vital signs, weights, and other duties per state regs and policies.
• Provide for or assist residents with activities of daily living (ADL) including bathing, oral care, toileting, dressing; perform restorative nursing techniques and ROM; offer prompts and support.
• Encourage residents to participate in activities; assist with transportation to activities and accompany as needed.
• Assist with transfers and ambulation using proper body mechanics and equipment; provide transportation to appointments.
• Respond to urgent calls promptly and perform other duties as assigned by supervisor.
