The role emphasizes patient care, safety, and compassionate interaction with clients and families. Responsibilities include assisting with daily living activities, personal hygiene, and care plan adherence, along with documenting changes and maintaining confidentiality.

Qualifications

• State licensure or certification as required by state.
• One year relevant experience preferred.
• Current BLS (Basic Life Support) certification.
• Current PPD or Chest X‑ray as required by program.
• Knowledge of child growth and development.
• Ability to coordinate services between home, school, and community resources.
• Strong communication skills; English proficiency may be required.
• Must be at least 18 years of age.
• Computer proficiency required.

Responsibilities

• Provides day‑to‑day care and support per plan of care.
• Performs health care tasks and personal hygiene services.
• Assists with transfers and ambulation.
• Coordinates services between home and school and other resources.
• Maintains client confidentiality and HIPAA compliance.
• Reports changes in client status as directed by supervisor.
