Job summary

Active Solutions ALF is seeking a compassionate caregiver to provide exceptional support to residents across care settings in Albuquerque. You will assist with ADLs, monitor health, manage medications, prepare meals, and support memory care needs with dementia-specific techniques.

The role emphasizes patient safety, HIPAA compliance, and collaboration with healthcare teams and families to deliver holistic care that enhances quality of life for seniors and individuals with disabilities.

Qualifications

• Experience in home health, hospice, nursing home or assisted living is preferred.
• Knowledge of ADLs, medication administration and vital signs monitoring.
• Ability to follow care plans and HIPAA guidelines.
• Ability to perform heavy lifting when necessary and work in a team.
• Experience with memory care needs such as dementia is desirable.

Responsibilities

• Assist residents with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and mobility support.
• Monitor residents for changes in condition or behavior and document observations.
• Administer medications and manage care plans per provider instructions.
• Prepare nutritious meals and assist with meal consumption as needed.
• Support memory care residents using specialized techniques for dementia care.
• Operate and monitor medical equipment and ensure patient safety.
• Maintain clean and safe environment with light housekeeping and sanitation.
• Provide emotional support and companionship to residents, fostering a caring atmosphere.
• Ensure patient confidentiality and HIPAA compliance at all times.
• Collaborate with healthcare teams, social workers, and family members to deliver comprehensive care.
