Sapphire Health Services, LLC is seeking a dedicated Caregiver to support residents in our community-based facility in Clatskanie, Oregon. You will assist with daily activities, promote independence, and provide compassionate companionship to residents, working closely with families and the care team.

The role emphasizes safety, confidentiality, and collaboration, with a day shift and on-site duties. A high school diploma is required, and prior caregiving experience is preferred.

Qualifications

• High school diploma or equivalent required.
• Experience in caregiving, home care, or community-based care preferred.
• Ability to work independently and as part of a team.
• Ability to perform physical tasks such as lifting, bending, and assisting clients with mobility.

Responsibilities

• Provide assistance with activities of daily living (ADLs).
• Monitor and report changes in residents' physical or mental health.
• Assist with serving resident meals and providing companionship.
• Maintain accurate records of care provided, including daily logs and progress notes.
• Maintain resident confidentiality and safety.
• Encourage participation in activities promoting physical, mental, and emotional well-being.
• Support independence in daily routines and decision-making.
• Identify and address potential hazards to ensure a safe environment.
• Respond to emergencies following established protocols.
• Collaborate with care team and families for coordinated care.
• Communicate care plans clearly with residents and families.
