Qualifications

• Be able to make independent decisions.
• Deal tactfully with residents, families, and staff.
• Capable of working with ill, disabled, elderly, and emotionally upset people.
• Communicate effectively with supervisors and team members.
• Maintain confidentiality of resident information.
• Knowledge/proficiency of facility EHR.

Responsibilities

• Provide residents with routine daily care and medication services per the assessment and service plan.
• Assist with activities of daily living and mobility while maintaining resident dignity.
• Observe and report changes in resident condition to the nurse promptly.
• Follow infection control and safety procedures and report unsafe conditions.
