Responsibilities

• Assist with ADLs and IADLs for clients in home setting.
• Establish effective communication with clients and family.
• Perform tasks as assigned in personal care service plan.
• Help maintain a clean and organized environment.
• Remind clients to take self-administered medications.
• Report changes in client condition.

Skills

Professional experience (CNA, HHA, PCA)

Alzheimer's/Dementia experience

Valid Driver's License
