Job summary

Clarity Care, Inc. is seeking an Activity Support Provider to assist with the Day Services Program in Green Bay. Starting pay up to $18/hr based on experience, with immediate hiring and no experience required.

You will support members in daily living, plan engaging activities, help with meals, and ensure safety and regulatory compliance while promoting independence and community involvement.

Qualifications

• Must possess a high school diploma/equivalent and/or related work/life experience.
• Must be at least 18 years of age.
• Must possess a valid WI driver’s license, and ability to meet agency driving standards.
• Must provide proof of auto insurance.
• Must pass a periodic WI Caregiver Background Check including DOJ checks.

Responsibilities

• Support members in community and recreational activities.
• Assist members with physical, social, and personal needs.
• Guide members in achieving personal goals and decision-making.
• Create fun and engaging activities tailored to member interests.
• Prepare and serve meals following dietary needs and preferences.
