A Place at Home - The Woodlands is seeking a compassionate caregiver, CNA, PCT, or Direct Support Professional to support a client in The Woodlands, TX. You will provide fall prevention, personal care, meal prep, and daily living assistance.

You will monitor medications, maintain records, and coordinate with family and healthcare providers to ensure safety and well-being, while adapting care plans as needs change.

Qualifikationen

• Proven experience in caregiving with a focus on fall-prevention.
• Excellent communication and interpersonal skills.
• Ability to handle challenging behaviors with patience and empathy.
• Reliable, compassionate, and dedicated to improving clients' quality of life.
• Current CPR and First Aid certification preferred.
• Valid driver’s license and reliable transportation required. Our clients need to be provided transportation to outings, doctor's appointments, etc.

Aufgaben

• Provide compassionate and attentive care to clients.
• Assist with daily living activities, including personal hygiene, grooming, and meal preparation.
• Monitor and manage medication schedules as prescribed by healthcare professionals.
• Implement and adapt care plans to accommodate changes in the client's condition.
• Engage clients in activities and exercises designed to stimulate cognitive function and promote well-being.
• Maintain accurate and confidential records of care provided and report any changes in the client’s condition to the supervisor.
• Communicate effectively with clients, family members, and healthcare professionals.
• Ensure a safe and clean-living environment for clients.
