Qualifications

• Strong communication skills to interact with clients and families.
• Ability to lift and assist with mobility needs.
• Compassionate, patient and empathetic approach.
• Direct support certification or related social work experience is a plus.

Responsibilities

• Assist clients with ADLs such as bathing, dressing, grooming, and meals.
• Provide patient observation and monitoring to ensure safety and well-being.
• Administer medications as prescribed and maintain care plans.
• Support mobility with equipment like Hoyer lifts when necessary.
• Engage clients in social activities to promote mental stimulation.
• Maintain cleanliness and organization of living spaces.
• Collaborate with healthcare professionals to deliver tailored care.
• Uphold HIPAA confidentiality and document patient information.
